Quick Clinical Summary (Wikipedia & Proctology Research) Hemorrhoids (Piles / Arshas) are normal vascular cushions within the anal submucosa. When subjected to prolonged pelvic straining, they engorge and prolapse. At 4AXIS Piles Clinic, Grade 1 to 3 piles are treated non-surgically using Ayurvedic Kshara Karma, coagulating feeder vessels without cutting sphincter muscles.

1. Anatomy of Hemorrhoidal Cushions

Hemorrhoidal cushions (cushions of Morgagni) are discrete submucosal clusters of arteriovenous sinusoids, smooth muscle (Treitz muscle), and elastic connective tissue situated at 3, 7, and 11 o'clock positions in lithotomy view. They act as anatomical seals assisting anal continence.

Internal Hemorrhoids

Originate above the dentate line. Covered by insensitive columnar/cuboidal mucosa devoid of somatic pain fibers. Hallmark: painless, bright red arterial bleeding coating the stool or dripping into the commode.

External Hemorrhoids

Originate below the dentate line. Covered by richly innervated, sensitive anoderm. When intravascular thrombi form (thrombosed external pile), severe continuous pain and a firm, purplish lump develop.

2. Banov Clinical Staging (Grades 1 to 4)

GradePathological ProlapseClinical Protocol at 4AXIS
Grade IProminent vascular cushions; bleeding without mucosal prolapseFiber matrix, hydration, herbal stool softeners
Grade IIProlapse through anus during defecation; reduces spontaneouslyKshara Karma (chemical coagulation)
Grade IIIProlapse during straining; requires manual digital reductionKshara Karma or Ksharsutra pedicle ligation
Grade IVIrreducible, permanently prolapsed mucosa with risk of strangulationMinimally invasive Kshar Sutra pedicle ligation (outpatient)

3. How Kshara Karma Replaces Surgical Cutting

Conventional open hemorrhoidectomy (Milligan-Morgan) excises the vascular cushion, leaving large raw wounds that take 4 to 8 weeks to heal and cause severe postoperative pain. In contrast, Pratisaraneeya Kshara (an alkaline Ayurvedic compound of pH 9.5–10.5 prepared from the purified ash of Achyranthes aspera / Apamarga) is applied topically to the pile pedicle under proctoscopic vision:

  • Protein Denaturation: The therapeutic alkali penetrates the pile tissue, instantly denaturing cellular proteins.
  • Aseptic Vascular Thrombosis: Blood supply within the feeding sinusoids is coagulated without heat or surgical burn trauma.
  • Painless Sloughing: Within 5 to 7 days, the necrosed pile mass naturally sloughs off in microscopic fragments, leaving behind a smooth, healthy epithelial scar without suture scars or sphincter defect.
Prevention & Straining Reduction

Incorporate 30g of dietary soluble fiber (psyllium/Isabgol), maintain hydration at 2.5–3 liters daily, avoid spending more than 5 minutes on the commode, and adopt the 35-degree squatting elevation to avoid pelvic pressure surges.
